Thank you for your interest in applying for terms with Lumin Fabrics! To apply, please fill out the form
below. Be sure to have your state Tax License Number (or resale license number) handy.

After you have submitted the required information, you will receive an email notification of the status 
of your request within three business days. 

Please note that all first-time orders must meet a $1000 minimum order amount and must be paid 
by credit card.
